Si su nombre legal actual es diferente del nombre en su certificado de nacimiento civil (por ejemplo, su nombre de soltera), se necesita una prueba legal del cambio de nombre.
Prueba legal = Acta de matrimonio o certificado de unión civil, divorcio u orden judicial que vincule el nombre nuevo al nombre anterior. Pasaporte de los EE.UU. no es una prueba de cambio legal de nombre. Un decreto de divorcio puede ser utilizado como autoridad para dejar de usar un nombre anterior, si contiene el nombre y el nombre anterior y permite un retorno a la utilización de la denominación anterior.

Name on your civil birth certificate is different from the legal name

If your name on your civil birth certificate is different from the current legal name, then you must show legal proof of the name change. Legal proof = Certified marriage or civil union certificate, divorce decree or court order linking the new name with a previous name. U.S. passport is not proof of legal name change. A divorce decree may be used as authority to resume using a previous name only if it contains the new name and the previous
name and permits a return to use of the previous name.
